Care Initiatives is hiring a hospice registered nurse case manager for Albia. The registered nurse case manager is responsible to coordinate all interdisciplinary team services provided to patients/families/caregivers. The registered nurse case manager renders nursing services to Care Initiatives Hospice patients. Responsible to oversee licensed practical nurses assigned to Care Initiatives Hospice patients; responsible to establish and manage hospice aide/hospice homemaker services provided to patients/families/ caregivers. Functions according to operational policies and procedures to meet patient/family/caregiver needs in compliance with federal, state and local requirements, Care Initiatives Hospice Policies and Procedures, and Care Initiatives Hospice Clinical Team Procedures.

Care Initiatives is hiring a hospice billing specialist for West Des Moines.The Hospice Billing Specialist is primarily responsible for all hospice-related billing activities, assuring appropriate recording of transactions, and routine monitoring of accounts receivable to hospice care activities. This full-time non-exempt position includes 40 hours/week Monday - Friday and entails: data entry, claims creation, ICD 10 understanding, payment posting, A/R management, answering provider questions, HIPPA compliance and more.

The RN is responsible for identifying and coordinating care for terminally ill patients and their families in homes, skilled nursing facilities or residential care facilities. Frequency of patient/family contacts will be at the discretion of the RN who is the patient case manager responsible for leadership and oversight of the entire interdisciplinary plan of care. The RN endeavors to utilize teaching, assessment and intervention sills to provide comfort care and maximize the quality of life for patients and families.
